Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PMJAY) is the worlds largest health insurance scheme launched in September 2018. It provides health coverage of Rs 5 lakh per family per year for secondary and tertiary hospitalization.

Visit pmjay.gov.in and click on Am I Eligible. Enter your mobile number to check if your family qualifies based on SECC database.
How to Apply
If eligible, visit the nearest empaneled hospital with your Aadhaar card. The hospital will verify your eligibility and issue your Ayushman card on the spot.
